Admissions Criteria
Medical Stability
Patients must be medically stable for discharge from acute care hospitals and do not require intensive monitoring or life-support equipment.
Housing Status
Eligibility is prioritized for individuals who identify as experiencing homelessness or lack a safe environment for post-hospital recovery.
Self-Care Capability
Individuals must be capable of performing activities of daily living (ADLs) independently or require only minimal non-clinical support.
Communal Participation
Patients must agree to adhere to the facility code of conduct and participate successfully within a communal, supervised living environment.
